Poté, co se doktor Watson oženil a odstěhoval od svého přítele Sherlocka Holmese, začal slavný detektiv holdovat kokainu více než je zdrávo a proto jej Watson na začátku snímku nachází jako zdrogovanou trosku. Holmes si navíc vsugeroval, že jeho bývalý učitel z dětství, profesor Moriarty, je vlastně "Napoleonem zločinu", ovládajícím celé londýnské podsvětí a neustále Moriartyho pronásleduje. Doktor Watson vyláká svého přítele do Vídně ke slavnému lékaři Sigmundu Freudovi, který zřejmě jako jediný může Holmesovi pomoci zbavit se závislosti.
